The Foundation will consider applications from organisations for initiatives, programs, projects, services, equipment, capital purchases or any assistance that will create positive health and wellbeing outcomes for the benefit of the community.

Document Downloads
Our Grant Application and Reporting forms are Microsoft Word Documents. We have chosen this format for ease of use. Please note that:
- You will need Microsoft Word or Microsoft 365 or equivalent in order to edit them
- Some antivirus products and browser security extensions (such as Malwarebytes) may flag them as suspicious as they are .DOC files. The documents have been extensively checked for any virus content (using the Virus Total service) and contain no macros, so please be assured they are safe to download.
Reporting Documents
To be used for multi-year, recurring grants.
A Progress Report must be submitted before the next instalment of the grant will be released.
To be used by all successful grant applicants.
A Final Report must be lodged within 60 days of the completion of the initiative.
